I noticed that if you put the diffuse white value too high (e.g. 255) the render fills with a sort of fog. Should it be doing this? This doesn't sound like a natural effect.

It isn't a natural effect, because it would be impossible in real life :)
Having 255 for RGB means the material is reflecting back the same amount of light it receives, so the light just bounces around loosing very little energy and Maxwell doesn't know what to do with it. There are no materials in real life that don't absorb some light. Even completely white walls should be set at around 245-250 max.
