For future reference (and not a bad idea when installing a new version) there is a plugin command you can use: Maxwell_ToolbarReset. The item in the main menu just shows all of the toolbars, but this command will delete the existing toolbar file and re-create it. So, if you have changed the toolbars, or messed up the images, you can use that command to bring things back.
