Rhino's normal-smoothing should work just as good as Studio's would, so I would say that your mesh is simply not fine enough. I'm not sure if there is a good source of information as to what would be appropriate settings - they are just quite different for different types of geometry - but 'smooth and slower' is still just a very general setting. Probably best to:
- select the object
- go to Object Properties
- in Render Mesh Settings, check Custom Mesh
- click the Adjust button
- in the dialog shown, click Simple Controls
- click Preview
At this point, you can probably identify where the distortion you're seeing relates to the actual mesh being generated. Try a few different settings using the density slider, and previewing those - you should be able to find a point where things look as you want them to.
About .mxs vs. .mxi, there is no File > Save As for .mxs, because the whole purpose of the plugin is to write .mxs files - that's what happens when you hit render. Where the .mxs is saved is specified in Scene Manager > Output Settings > Scene Path. The .mxs file holds all of the meshes and materials in the Scene - the .mxi file is a file which holds the actual image data that is produced by the renderer.
