It looks like 2009 has some weird caching scheme for viewport textures and doesn't rebuild them when you change something. However, it doesn't seem to be plugin-related, I tried it with a lambert material on an object in your scene and it has the same problem. You can force it to refresh by selecting "reload image file" on the texture node. I'll keep looking, maybe I can figure out what's going on.
The problem with the texture not showing up on the cube is that the cube has bad UVs (they are all collapsed to a single point). Apply a mapping to it and you'll see that the texture appears (don't forget to do a "reload image" after that, the cache thing happens here too).
The empty space in the AE is due to a limitation in the OSX user interface functions that Maya exposes (it doesn't happen on Windows). I had a workaround in place for it which was functioning at some point, but apparently it's back. Thanks for reporting it, I'll try to fix it.
