Key-Mapping-Schemes needed for ID-Project

Hi All,
I've posted this to CG-Talk - unfortunately nobody got back to me.
As Maxwell is available for many Subdivision-Based Modeling Programs I thought I should try my luck here too
For a Design- Proposal to a Manufacturer of Input-Devices for 3D-Modelling I am currenty comparing the Input-Schemes of Subdivision Surface Modellers as well as Digital Sculpting Programs.
I would like to illustrate typical Movements of the left Hand with a layered Graphic, while creating a simple piece of Geometry.
While I have used in the past and still use quite a few Programs myself I don't know or remember the exact mappings for all relevant subdivision based Apps out there.
Could you Guys and Girls around the World maybe help me out?
Basically I am interested in all Modelling-Functionality driven with the left hand by using Shortcut-Keys and Modifiers only.
What's happening with the Mouse/Pen-Hand, whether LMB or RMB are pressed gets disregarded.
Some of the larger Apps also offer several Mapping-Schemes, based on current work-context so that e.g. Ctrl+T in Rigging drives another function as in Modeling. I am however only interested in Geometry-Editing here.
Attached to this Posting you'll find a Screenshot of a .pdf I have set up. It contains editable Form-Fields, so one can write the respective Functionality directly onto the Keys.
You don't have to get crazy - I would be happy for the most basic 20 shortcuts or so. Shouldn't take long to fill in.
You can download the actual Form here (sorry, one can not add attachments in the Off-Topic Forum). I would be very glad if you could give it a short Spin and then sent it to polyxo [ät]web.de.
I would then add a note to this thread that the Form for your Program has aready been filled in, so that no work gets done twice.
Here's a list of SubD-Programs of which I still need the (Default) Mapping Schemes:
3DS Max, Maya, Softimage, Modo, Cinema4D, Lightwave, Houdini, Blender, Hexagon... ...(I already have Silo)
Of the better known Sculpting-Apps I only would mostly need the Scheme of Mudbox (I already have Zbrush and 3DCoat).
But you are of course welcome to add any Application I forgot.
