how are they created? you mean the HDRI or the QTVR?

The HDRI is created by taking "with a tripod" pictures with different exposures and then combining them in your favorite application like Photoshop CS2 or HDRshop (free)

The QTVR, has many ways to do it from taking pictures of a mirror ball to using fisheye lenses and then merging them together in programs like Ptgui. After you created your 360 panorama you'll need a program like pano2qtvr to convert them into qtvr.

If you are doing a HDRI it means that you need to create identical 360 panoramas of a place with different exposures and then combining them on PS CS2 so you can use it in Maxwell as a light, reflection, background, etc
