A quick, basic question about a Maxwell over photo composite

In the above image created by Gokhan in this lemonade thread
and in scenes like it created by other people, what am I looking at exactly regarding lighting and compositing?

Is the photo rendered in at render time, or composited later? I assume it's in the render, because of the refraction.

Is the image driving the lighting? (I haven't had occasion to use IBL yet)? Is it an emitter? What is its bit depth? 32 bit HDR? Or just an LDR image?

I tend to put live action (people) over Maxwell CG backgrounds, so I'm a little rusty on best methods for putting Maxwell CG over live action.

Oh, everyone else, (and i do mean everyone) has been using IBL for around 15 years!....

When you get around to it, you'll find out all the caveats of the infinite sphere...

That's why I always avoided it. I never quite saw the point of using a sphere at infinity to represent close interaction. I prefer to match-model the scene geometry and camera-project the photography onto it. Then close walls and floors really are close walls and floors.

So is that what the image above is? An IBL sphere?
